Obtaining a Visa Eligibility Document

Degree Programs:

After you have been admitted to Cal Poly for a degree program through Admissions submit the Visa Information Form for Degree Students. Send no financial documents via email. Use the form to upload all required documents.

VISIT Program:

After you have been accepted to the VISIT Program, please complete the Visa Information Form for Degree Students to request your I-20 or DS-2019. The I-20 is used to apply for an F-1 visa and the DS-2019 is used to apply for a J-1 visa.

If you are a sponsored student (receiving tuition from an organization) to attend the VISIT program, then you will receive a DS-2019 to apply for a J-1 visa.

Exchange Programs:

After you are accepted to an exchange program, you will be requested to provide additional information so Cal Poly can issue the Form DS-2019. The DS-2019 is used to apply for a J-1 exchange student visa. For application instructions and what to do after acceptance, go to the International Exchange Programs web page.
